JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Responsible for clinical assessment, interventions and, crisis management for children and their families in Northeast Philadelphia. The UCC program operates Monday through Friday between the hours of 11:00am 7:30pm. Services include assessment, crisis intervention, parent education, and linkages to behavioral health services as indicated.

PHYSICAL DEMANDS:
Minimal in nature

QUALIFICATIONS:
Masters degree in the human services field from an accredited college or university; At minimum, 2 years verified, paid, post-masters therapy experience with children/youth and training and experience in family therapy; Excellent interpersonal skills

SPECIFIC DUTIES:
1. To provide crisis management that is coordinated and integrated with the Interdisciplinary treatment team of the UCC.
2. To provide telephone crisis and triage coverage during hours of operation in order to arrange for assessment, make referrals to appropriate services, or provide education and support of ongoing care.
3. Engage the youth and family upon the visit to develop an understanding of the events that led to the crisis from everyones perspective orient the family on the role and UCC.
4. Complete crisis assessment of the child / youth and family obtain relevant information and develop a plan for interventions, including conflict resolution and crisis management.
5. Collaborate with the psychiatrist regarding assessment needs.
6. Utilize American Society of Addiction Medicine (ASAM) treatment criteria with youth who are in need of substance use services.
7. Utilize approved evidence based tools and practices including SAMHSA crisis guidelines.
8. To complete or assist others in completing the involuntary commitment petitions.
9. To follow all outlined procedures regarding the initiation of a commitment.
10. To safely deescalate crisis, only using approved therapeutic holds when a youth is engaging in behavior that presents a danger to self or others.
11. Collaborate with the crisis manager on resources and linkages needed and assure the linkages are in place.
12. Coordinate transportation to CRC at 7:30pm, if indicated.
13. To continue your shift in the UCC until your replacement arrives or until your supervisor gives you approval to leave.
14. Participate in training and on going discussion regarding the norms and values of the variety of ethnic and cultural groups in Northeast Philadelphia.
15. Obtain language/ translation services as needed to work effectively with youth and families.
16. Review and assure compliance with all documentation requirements.
17. Assure any follow up appointments are scheduled at a time and place convenient for families, which includes the provision of evening and weekend appointments in the home and community.
18. Complete all required documentation timely and legible manner and in accordance with PATH policies and procedures.
19. Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.
