Summary:Monitor and coordinate the pre-admission, admission and discharge planning needs of patients. Maintain compliance with applicable policy and procedures and compliance with state regulations. This is a highly visible position involved with presenting the treatment program to potential clients, referral sources and the community at large. Strong organization skills are required to maintain to coordinate welcoming, documentation completion, orientation and discharge planning activities of patients. The Navigator ensures the patient is contacted prior to admission, verifies insurance as assigned, completes admission consents and orients the patient, completes the clinical Biopsychosocial. During the course of treatment the Navigator assists the clinical team and patient in identifying discharge resource needs, completing sober living applications, setting up appointments and the like. All activities are documented in the EMR in a timely fashion.

Duties and Responsibilities
  • Assurance of compliance with state, federal, accreditation boards laws and program policies.
  • Manage desk time for welcoming potential patients by making contact to support treatment decision.
  • Review and monitor all intake communication from the call center on a daily basis.
  • Communicate any red flag indicators of potential admissions to the leadership team.
  • Ensure benefit verification for all scheduled appointments for the day, to include monitoring and approval of financial charts. Confer with billing company on all benefit verifications and issues.
  • Complete admission financials on a daily basis to include confirmation of all insurance authorizations for all admissions and transitions.
  • Contact all no-show appointments 30 minutes after missed appointment and complete appropriate follow-up.
  • Maintain daily register sheet entering admission and discharge information as applicable.
  • Provides appropriate and timely discharge planning from the facility, working closely with Utilization Review staff as to maximizing reimbursement and maintaining appropriate census at each level of care.
  • Maintain effective communication with clinicians, admissions, physicians, nursing, patients, families and others related to discharge planning
  • Documents discharge planning activities in EMR and Register as well as communicating status to patient's counselor.
  • Functions as a member of a multidisciplinary team.
  • Participates in continuing education programs to maintain skills
  • Maintain effective working relationships with facility staff.
  • Responsible to abide by all policies and procedures, including limits of confidentiality of patient information requirements of Pinnacle Treatment Centers.
  • Knowledge and awareness of CARF standards as it pertains to admission and discharge process.
  • Provide exceptional customer service to all internal/external customers with rapid response to inquiries and concerns & support facility internal and external customer service standards
  • Adhere to all safety policies and safe work practice
  • Implement other duties as assigned by the Executive Director.

Skills and Abilities:
  • Bachelor's Degree in a human service field (minimum), Master's preferred.
  • Minimum of three years of experience in substance abuse disorder treatment field.
  • Strong communication and computer skills
  • Multi-tasking ability, organizational skills, ability to work independently, problem solving techniques, and strong interpersonal skills.
  • Philosophy of treatment that is compatible with facility philosophy.
