Wellness Liaison
Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Wellness Liaison to join our team at Inwood.
The Wellness Liaison is responsible to link clients to Sun River Health and create easy access to services. The Wellness Liaison works closely with the account management team to connect clients to services and plays a key role in creating an excellent experience for the client. The Wellness Liaison is responsible for providing outreach and education on health home services and to coordinate intake and enrollment into Health Home program. The position requires strong interpersonal skills to make connections and build relationships with the target patient/client base. The Wellness Liaison works collaboratively with other departments as well as community-based agencies with the purpose of making linkages to health services for community members. The Wellness Liaison may work externally at Community Partner sites, health fairs and events or may be based in the Sun River Health centers. The Wellness Liaison as an Ambassador of Sun River Health whose activities reflect the mission and value of the organization and will provide outstanding customer services.
Essential Duties and Responsibilities for a Wellness Liaison:

Serves as liaison between the clients, providers, and account management staff

Participates in planning and coordination of outreach and educational events

Assists clients in securing appointments for HRHCare Health Center services.

Provides outbound reminder calls to clients prior to their scheduled appointments.

Conducts Health Home pre-eligibility screenings and intakes for eligible clients.
Achieves intake monthly targets that are set on annual basis and adjusted quarterly as needed. -
Aide with client outreach of clients, education of HRHCare services to community with a focus on linkage to Health Center and Health Home.

Assists in arranging transportation when needed.

Assists with health insurance application when needed.

Responsible for the complete, accurate and timely entry of data into required database, including but not limited to demographic information, insurance information, assessments and progress notes.

Responsible for the initial entry, verification and maintenance of information in required Database, BTQ Portal and any additional database specified.

Provide necessary documentation and information to health home base in each borough

Participates and attends Community Events such as health fairs; conducts on-street marketing of flyers and service information pamphlets

Provide support of Health Home team by utilizing Sprinter/Mobile Van in specified locations on schedule to conduct Health Home Intakes for health fairs, special events and locations with limited onsite capacity.

Acts as initial service ambassador on the promotion and education to referral sources, clients and community members of all HRHCare services while providing the highest quality service.

Supports HRHCare’s mission, values, strategic goals and high standards of customer service.

Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.

Participates in team meetings.

Performs other duties as assigned by Supervisor.
Minimum Education Requirement: High School diploma/GED
Minimum Work Related Experience: 2-4 years
