Patient Care:
- Responsible for the cost-effective assessment, planning, implementation, and evaluation of case management activities including utilization review and transition of care
- Coordinates the overall interdisciplinary plan of care for a patient from admission to discharge
- Assesses and documents patient's progress throughout hospital stay
- Evaluates treatment goals and makes modifications, as required
- Evaluates patient satisfaction and quality of care provided
- Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is efficiently and effectively provided
- Works with agencies and service providers to facilitate case coordination and information sharing
- Provides education and literature to healthcare team, patients and/or family members
- Assists patients/family members in making informed decisions
- Facilitates patient/family access to health care, schools, and community resources
- Monitors and maintains departmental budget
- Travels to outreach locations to meet with patients/families in order to coordinate services
- Consults with admitting provider and patient financial services
- Updates patient's medical records as required
- Utilizes Electronic Health Records (EHR) and demographic information to identify patient needs and intervention
- Prepares appropriate reports as required
- Adheres to NMHS/NMMC Policies/Procedures/Guidelines
- Complies with applicable Local/State/Federal policies/procedures/guideline/regulations/laws/statues
- Associates Degree in Nursing, required; Bachelor's Degree preferred
- Licensed as a Registered Nurse by the Mississippi Board of Nursing; required
- Minimum of 5 years clinical and/or healthcare experience; required
- Excellent organizational and communication (written and verbal) skills; required
- Knowledge of various payer sources, federal/state laws/regulations, and cost containment; required
- Certified as an Accredited Case Manager (ACM); desirable
- Excellent interpersonal skills; required
- Demonstrates ability to care for a patient population from pediatric to geriatric; required
