JOB SUMMARY
The , reports to the Vice President, System Care Management, with indirect reporting to ambulatory executive leadership and is responsible for leading and overseeing all Care Management functions within the ambulatory setting. This role will serve as an expert resource in ambulatory care management and provide leadership for the Ambulatory Care Management team, including nurse case managers, social workers, community health workers, and care management coordinators.
This role requires an extensive background in the ambulatory setting including the critical thinking and decision making that arise during ambulatory patient care. Specifically, this role requires an extensive knowledge base of the considerations associated with high risk patient care, management of patients with multiple comorbidities, and/or support of those that are at increased risk of emergency/acute care admissions or readmissions through care management and coordination with multidisciplinary care teams, including primary and specialty care, patient support systems, and community resources.
This role will champion ambulatory care management operations, leading initiatives across ambulatory settings based on clinical care management best practices, incorporating lean change management tools and standard work to improve patient care, satisfaction, and outcomes and support the organization¿s strategic plan. This role will partner in care management with clinical, operational, and business stakeholders across the system, including, but not limited to, inpatient care management operations at Ben Taub and LBJ Hospitals and Outsourced Medical Services as well as partners across the regional healthcare systems and community. As a system leader and care management expert, this role will have high visibility and accountability to executive health system leadership, physicians, and care team members and will be expected to quickly acclimate to the Harris Health System family by establishing relationships, proactively identifying needs, influencing decision making, implementing innovative and transformative change, monitoring outcomes, and demonstrating exceptional team building and communication skills.
In the course of overseeing daily operations, this role will be responsible for leveraging information technology and data for monitoring and reporting on budget, staffing, and key performance goals and will use data-driven decision making to optimize workflows and improve throughput. As a leader, this role will lead with confidence, empathy, and integrity to build a team-oriented collaborative culture focused on patient care through appropriate resource utilization across the continuum of care, connections with community resources, disease-specific education, and other care management supports as appropriate to improve patient outcomes and satisfaction while reducing acute care needs.
Minimum Qualifications
Degree: Graduation from an accredited School of Nursing with a Master's degree in Nursing or related field (Healthcare Administration, Business Administration, or Public Administration) required. Holds at least one (1) of the following in Nursing: Bachelors, Masters, or Doctoral degree.
Licensed to practice professional nursing in the state of Texas
or
Graduation from an accredited school of Social Work with a Master's degree in Social Work
Licensed Clinical Social Worker (LCSW) preferred
Holds a current licensure in the State of Texas
Related specialty certification is required.
Accredited Case Manager certification (ACM or CCM) to be completed within two (2) years if not held at the time of appointment.
Work Experience: Five (5) years of clinical or social work experience in a challenging healthcare environment plus at least five years of experience in care or case management. Ambulatory care or case management experience preferred.
Management Experience: Five years related management/leadership experience is required. Hospital leadership experience in a complex health system managing multiple departments or facilities preferred. Specific experience in managing Care Management in ambulatory settings to include care coordination, utilization management, transition planning, and social work preferred.
