Bilingual Case Manager

Bilingual Case Manager for Community, School-based & Peer-to-Peer programs. Providing Case Management, Program Coordination, and bilingual Big Buddy recruitment support in the Community, School-based, and Peer-to-Peer programs • Maintaining an average caseload of 30 Buddy Pairs within the Community and School-based Programs • Coordination and supervision of one Peer-to-Peer program site with the assistance of one other program staff member • Cultivating trusting, constructive relationships with families and youth via an intake process and on-going case management, as well as overseeing required paperwork and documentation that goes along with such duties • Screening and making strong and compatible matches with Aspen/Basalt/Carbondale youth and volunteers in the Community, School-based, and Peer-to-Peer Programs • Coaching and supporting adult and high school volunteers in their role as mentors to youth • Cultivating relationships within the Latino/a/x community amongst individual and partner organizations, including hosting events, presenting at businesses and organizations, meeting with potential volunteers, attending local networking events • In conjunction with the Recruitment Manager, identify qualified volunteer candidates for the role of Big Buddies and brainstorming new recruitment activities, methods, and events • Ongoing support and case management o of children waiting for a mentor • Developing and maintaining relationships with schools (including Peer-to-Peer School Liaisons, and attending school meetings),other youth organizations, community leaders, and various organizations with whom we collaborate for youth referrals, volunteer recruiting, case management, and special projects • Remaining up-to-date, educated, and knowledgeable in the fields of mentoring and youth services in order to support and coach volunteers in their role as mentors • Coordinating services and supporting team with pertinent outreach pertaining to Community and School-based Programs (empowerment scholarships, mentor, mentee and parent trainings, counseling referrals, and other agency referrals) • Communication with Big Buddies to ensure consistent and quality participation and attendance • Engagement and attendance in team meetings, including: Case Manager meetings, Recruitment team meetings, Peer-to-Peer team meetings • Planning, preparation, and facilitation of Buddy Pair activities during Peer-to-Peer programming • Maintaining and updating on-line and written data files on an ongoing basis • Maintaining client confidentiality • Regular attendance at appropriate Health & Human Service Agency meetings • Attending Buddy Program sponsored activities/events and assisting with special projects as they arise • Support of events, including: Bash for the Buddies, Boogie’s Buddy 5 Mile Race, Gingerbread House, recruitment events
