To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Utilize the Boston University Psychiatric Rehabilitation model to assist residents in rehabilitation goals and developing the skills and resources required to meet defined goals.


Masters degree in the human services field. Experience in working within a human service program. Good interpersonal skills and effective communication skills.


Minimal in nature but must be able to perform crisis management interventions including restraints once training is successfully completed.

1. Provide individual, group and family therapy at least weekly for all residents
2. Ensure that delivery of clinical treatment is congruent with approved treatment models
3. Be flexible in scheduling clinical treatment to meet resident and family needs
4. Maintain on-going communication and collaboration with family members, to include scheduling, follow up and documentation of each therapeutic leave
5. Complete biopsychosocial evaluations for potential admissions as assigned
6. With the resident and treatment team, develop discharge plans and arrange resources defined in the discharge plan
7. Complete all documentation required for continued stay and communicate authorizations to PATHs QI department in a timely manner
8. Participate in CBH phone reviews with program director or program manager as requested
9. Complete individual service plans and individual treatment plans in a timely manner for all residents
10. Participate in weekly treatment team meetings with the RTF Psychiatrist
11. Consult and collaborate with all team members on a regular basis
12. Provide psychiatric rehabilitation services for all residents
13. Ensure the delivery of psychiatric rehabilitation services is congruent with the Boston University Model
14. Assess decision making readiness regarding environment choices in living, learning, working and social areas
15. Develop an Overall Rehabilitation Goal (ORG) and rehabilitation plan based on the ROG with each resident
16. Implement and provide interventions to support attainment of the residents ORG
17. Provide skill development training such as travel, interviewing, opening bank accounts and social skills for residents
18. Provide individual meetings with each resident regarding their ORG and rehab plan
19. Liaison with school, employers, job coaches and other support individuals involved with each resident to assist and resolve any issues impeding goal attainment
20. Assure that each resident has been linked to a pre-vocational/vocational, education, volunteer, or paid employment opportunity
21. Assure that residents are linked to MRS if indicated and maintain communication with MRS supports coordinator
22. Assure each eligible resident has social security benefits and that change of address information is provided as indicated
23. Provide program coverage as assigned
24. Participate in team meetings, case conferences, stakeholders meetings and other assigned meetings and complete any related documentation
25. Provide crisis intervention and management as indicated
26. Assist in the development of community activities for residents, to include weekend activity planning
27. Contribute to quality improvement/outcome measurement tracking and data gathering as assigned
