To further the spiritual and religious mission of the Catholic Church's call to minister to the poor and vulnerable by screening, assessing, and coordinating needed services with a focus on housing. To foster a welcoming atmosphere where clients are treated with dignity and respect. The Housing Case Manager provides intake, case management, and extensive follow up with clients to determine continued housing; working cooperatively with other agencies to link individuals to financial services and community resources that meet their expressed needs; and providing direct services and accurate data entry to track and record contacts.

Essential Duties/Responsibilities

  • Conduct intake screenings and assessments for households who are seeking emergency services, including homeless prevention supports, in order to maintain current housing situation or rapid re-housing resources.
  • With the client, create a housing stabilization plan and schedule for follow up to assess progress toward goals.
  • Follow up client contacts to assess goal attainment and need for further referrals.
  • Connect to other professionals and resources (internal and external) to create a network of ongoing support with the client.
  • Cultivate and maintain working relationships with landlords and provider organizations for referral and follow up support.
  • Submit and maintain accurate financial invoices and reports.
  • Maintain accurate databases (HMIS) of client demographics and services provided. Use appropriate forms to collect demographic program information for accurate documentation of case plans and to produce reliable program statistics and reports.

Skills & Competencies

  • Ability to work with people in crisis with a calming, positive attitude.
  • Ability to use sound judgment within defined policies and procedures to determine appropriate action.
  • Experience with training volunteers to follow agency protocols in adherence to agency policies.
  • Knowledge of community resources and systems.
  • Knowledge of social issues such as poverty, gender bias, domestic violence, alcohol and other drug abuse, child maltreatment prevention, cultural diversity, etc.
  • Proficiency with computer software including Microsoft Office suite.

Work Environment

  • A majority of the assigned duties are performed in an office environment.
  • The employee is subject to both inside and outside environmental conditions.

  • Bachelor's degree in human services, social work, or related field preferred.
  • Minimum requirement of a combination of experience and Associates degree in social work, human services, or related field.
  • Two years of full-time experience working with direct client/family interaction.
  • Ability to work effectively with people representing a diverse range of socioeconomic levels while being sensitive to cultural differences.
  • Sensitivity to issues involving poor and homeless populations.
  • Demonstrated ability to apply culturally appropriate skills in interactions with clients, co-workers, volunteers, partner agencies, and other community members.
  • Excellent communication (verbal and written), problem solving, listening, and interpersonal skills, with the ability to form and maintain positive work relationships.
  • Ability to work efficiently, effectively, and multitask in a fast-paced environment.
  • Ability and willingness to work within the established structure of Catholic Charities.
  • Occasional evenings and weekends are required to accommodate meetings and special events.



Position Classification


Full Time: 40 hours per week

Schedule: Monday - Friday 8:00 am - 5:00 pm
