POSITION PURPOSE

This position is grant-funded for a duration of 14 months, although the grant is anticipated to end in 2022, there may be an opportunity to extend employment following the end of grant period. The Community Health Worker (CHW) is a trusted, trained community member who promotes access to services, provides health education, supports care delivery and promotes advocacy. The goal of the CHW is to improve overall health outcomes, prevent hospitalization, and increase engagement in services. As an integral part of a multi-disciplinary team, comprised of internal staff, primary care and community partners, the CHW will assist at the individual level.

ESSENTIAL JOB RESPONSIBILITIES

Essential Position Functions
  • Working with the member and healthcare team, to develop a plan for the member to increase patient activation, access and appropriate use of service, working collaboratively with care coordinators/case managers as well as the members broader healthcare team in developing and implementing care plans that address members whole health needs
  • Document services, interventions, etc. in Case Management Platform in accordance with established standards
  • Identify and reduce or eliminate barriers to care, reporting identified barriers to the care coordinator overseeing their care plan
  • Demonstrate an on-going understanding and current knowledge of public benefit programs, specifically Temporary Assistance for Needy Families (TANF); including awareness of social and economic conditions and their effects upon individuals
  • Command strong working knowledge of the TANF pilot program resources and workflows so as to educate and support members through the processes involved in the different service delivery systems they will encounter
  • Develop and maintain a trusted relationship with the member by: showing patience, being open-minded, nonjudgmental, compassionate, flexible, responsible, supportive, and demonstrating follow-through; being available when needed; helping member learn new skills; meeting regularly with members as needed
  • Assisting and educating members in enrolling in health and social services programs (SNAP, WIC, health support systems); helping member access community services such as transportation, shelter, food, phone etc. Helping member learn skills to access enroll in and utilize services and resources; helping member make and get to appointments (excluding transporting at this time); helping member overcome resistance to using services; working with team to identify tools and resources to use in mentoring; reinforcing success, providing corrections and feedback
  • Act as liaison between member and PCP when appropriate, accompany member to appointments
  • Communicate, establish and maintain cooperative and productive working relationships with internal and external stakeholders
  • Assist members in accessing telehealth behavioral health treatment, including obtaining a loaner phone or other device, assist member in applying for Lifeline Medicaid phone program, Data collection including tracking of phones, members assisted
  • Creating connections between vulnerable populations and healthcare systems
  • Facilitating healthcare and social service system navigation
  • Referring members to case management who are in need of managed care or transitional care for vulnerable populations
  • Reducing social isolation among patient populations
  • Educating health system providers and stakeholders about community health needs.
  • Providing culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition.
  • Advocating for underserved individuals to receive appropriate services
  • Collecting data and relaying information to policymakers to inform policy change and development
  • Providing informal counseling, preliminary health screeningsusing CHW assessment form/social determinants of health, and referrals
  • Participate in multidisciplinary meetings to inform providers, provide advocacy, and represent members needs through multi-system intervention
  • Building capacity to address health issue(s)
  • Perform other duties and projects as assigned

Working Conditions
  • This majority of work completed with position is done in the field and includes some office work. Field work requires travel to clients in a variety of settings, homes, facilities, etc. of varying level of cleanliness and repair, and requires driving an automobile owned by the employee.
  • Office work will be conducted utilizing general office equipment. Office environment may include substantial sitting, multitasking, computer work and is subject to noise and interruptions.
  • This position requires scheduling flexibility; some evening, weekend work, and call may be required. Contact with the public in home or office environments may risk exposure to irrational/hostile behavior, contagious diseases, or contact with domestic animals.

QUALIFICATIONS
  • High school diploma
  • Completion of Community Health Worker training approved by the Oregon Health Authority.
  • Experience caring for someone with a chronic condition and understanding issues that may prevent access or use of available services.
  • Ability to help develop a plan to address member identified issues.
  • Understanding and ability to maintain appropriate personal and professional boundaries while developing a close, effective relationship.
  • Ability to drive
  • Ability to demonstrate and learn technology skills related to the work needing to be done (word processing, spreadsheets, database, internet research, email, IM, phones, copiers, assessment programs like CAPS, Oregon ACCESS and PAM).
  • Ability to learn and apply policies, regulations, requirements, rules around mandatory reporting and privacy and confidentiality.
  • Ability to handle confidential information with discretion.
  • Excellent organizational skills including ability to handle multiple priorities and demands in a dynamic work environment.
  • Knowledge of basic managed care concepts and preferably the OHP.
  • Ability to adapt to fast paced environment and demonstrated flexibility in operating in different work settings.
  • Ability to work independently and be self-motivated by seeking input from peers and supervisors when needed.
  • Use professional oral and written communication while meeting deadlines.
  • Demonstrates cooperation and teamwork.
  • Valid drivers license with no more than one minor moving violation in the past 3 years; no at fault accidents and no major violations
  • Automobile liability coverage or access to an insured vehicle

PHYSICAL DEMANDS

Typical office environment requiring standing, sitting, walking, bending, and lifting up to 25 pounds. May be exposed to patient and environment conditions such as unpleasant sights, smells and contagious diseases. Job Conditions: May be required to work or attend meetings after hours. May be on call through the use of a cell phone. Position involves exposure to people with a wide variety of health problems. May be called upon to work with difficult client and crisis situations.
