What is a Case Manager in the Assertive Outreach program?
Case Managers in Vail Place's Assertive Outreach Program provide intensive outreach to people with serious mental illness with complex needs who need support and services to maintain safety and stability in the community. Individuals may be facing hospitalization, homelessness, substance use disorders and/or chronic medical conditions. This program addresses their many needs in ensuring continuity of care, referrals to needed resources, care coordination, monitoring, discharge planning/transitions of care, housing support, and intensive goal planning.
Our innovative, collaborative service model aims to connect these individuals to vital resources, assist them to get the supports they need, and connect them with long-term services. We are seeking skilled case managers or service coordinators who have a passion for addressing complex needs and experience using a person-centered approach to engage clients in effective planning and problem-solving.
This position is temporarily remote, but will return to an in-person service model as soon as COVID-19 protocols allow. Applicants will be required to perform in-person work in the Twin Cities on a regular basis.
